1. A passport or other travel document that meets these requirements:

• be valid for at least three months after the scheduled date of departure from Ukraine;

• be free of any visas, marks, or stamps on at least two pages; 

• be granted for no more than ten years

2. A visa application form that has been printed and signed;

3. 1 photo of the applicant (35x45 mm);

4. Proof that the applicant has (or is entitled to) a valid medical/travel insurance policy covers Ukraine and the term of stay (the policy should cover at least €30 000 or similar coverage). Diplomatic and service passport holders are free from this requirement.

5. Confirmation of adequate cash to cover the expense of your stay (For example, a bank statement for the last two months, cash in Ukrainian hryvnia/other convertible currency, bank cards with current/savings account balance statements if the bank cards can be cashed in Ukraine, hotel/accommodation booking, a letter from the applicant's employer/sponsor confirming coverage of applicant's expenses in Ukraine, a return/transit ticket, etc.);

6. A receipt for a paid visa fee (unless Ukraine's legislation or an international treaty states otherwise).

1. A certified copy of the work permit from Ukraine's State Employment Centre. 2. A copy of the immigration permit issued by a local State Migration Service office in Ukraine;
